Leapfrog’s Health System Support subscription for multi-hospital health systems is designed to help Survey coordinators and health system leaders become in-house experts on the Leapfrog Hospital Survey and the Hospital Safety Grade and make it easy to monitor, compare, and analyze your hospitals’ Leapfrog Hospital Survey Results, Hospital Safety Grades, and Competitive Benchmarking scores.

The subscription gives you timely data for all your hospitals in a series of easy-to-use Excel workbooks, so you can quickly perform analysis, benchmark performance, build performance dashboards, and identify areas for improvement.

The subscription also includes a seat at Leapfrog’s monthly Hospital Webinar Series, so you can become an in-house expert on all aspects of the Leapfrog Hospital Survey and Hospital Safety Grade.

The support package includes the following:

Hospital Webinar Series (one registration per health system)

This webinar series is held monthly from March to December and includes monthly 30-minute office hour sessions to ensure that Survey coordinators and health system leaders have the information they need about the Hospital Survey, Safety Grade, and Benchmarking Scores when they need it. The monthly sessions provide a regular touch point for individuals who are often expected to be the in-house Leapfrog experts for their health system.

 

Leapfrog Hospital Survey Responses and Results

April Security Codes File: Excel workbook containing security codes for each of your hospitals. Facilitate access to the Online Survey Tool, check on their submission status in advance of deadlines, and view outstanding electronic data review warnings.
July June Submission Response File - 

Excel workbook containing a data dictionary and Survey responses submitted by each of your hospitals by the June 30 Submission Deadline.

 

June Submission Results File – Excel workbook containing data dictionary and the scored Survey Results for Surveys submitted by each of your hospitals by the June 30 Submission Deadline.

Perform data entry checks, compare to last year’s responses to ensure consistency, identify potential data reporting errors, investigate Data Verification Messages, and ensure Survey completeness prior to the August 31 Top Hospital Deadline and Data Snapshot Date for the fall Hospital Safety Grade. 
September

Mid-Cycle Response File – Excel workbook containing a data dictionary and Survey responses submitted by each of your hospitals by August 31 (deadline for Top Hospital and data snapshot date for the fall Safety Grade).

 

Mid-Cycle Results File – Excel workbook containing data dictionary and the scored Survey Results for Surveys submitted by each of your hospitals by August 31(deadline for Top Hospital and data snapshot date for the fall Safety Grade).

Perform data entry checks, compare to last year’s responses to ensure consistency, identify potential data reporting errors, and ensure Survey completeness prior to the November 30 Late Submission and Performance Updates Deadline.

 

Review Survey Results and identify opportunities to update and resubmit responses based on improved performance prior to the November 30 Late Submission and Performance Updates Deadline. 

February

Year-End Response File – Excel workbook containing a data dictionary and Survey responses submitted by each of your hospitals by January 31 (Corrections Deadline and data snapshot date for the spring Safety Grade). 

 

Year-End Results File – Excel workbook containing data dictionary and the scored Survey Results for Surveys submitted by each of your hospitals by January 31 (Corrections Deadline and data snapshot date for the spring Safety Grade). 

 

CPOE Report - PDF report containing order-checking category and overall scores for each of your hospitals that completed the Adult Inpatient Test. Report includes comparison cohort of other hospitals that use the same EHR.

 

Track performance over time and among your hospitals, set up dashboards and other benchmarking tools, create charts and graphs for presentations, identify gaps and set goals for the next year's Survey, and perform other analysis.

Leapfrog Hospital Safety Grades

Usernames and Passwords File – Excel workbook containing the usernames and passwords to the Safety Grade Review Website for all your hospitals receiving a Safety Grade each round

Facilitate access to the Safety Grade Review Website for all graded hospitals, review data used to calculate Safety Grades, and preliminary numerical scores. 
February & September

Spring and Fall Safety Grade Review File - Excel workbook containing a data dictionary, the data source, reporting period, measure score, and z-score for each measure used to calculate the Safety Grade, and the preliminary numerical score displayed on the Safety Grade Review Website during the 2-week Courtesy Review Period. File is sent out with the Safety Grade Calculator. 

Perform review of data displayed on the Review Website with publicly reported data from Leapfrog and CMS, review changes in measure scores, means and standard deviations, and measure weights from previous rounds, start analysis of measure score changes that may result in a letter grade change. 

March & October 

Spring and Fall Safety Grade Embargo File - Excel workbook containing a data dictionary, the data source, reporting period, and measure score for each measure used to calculate the Safety Grade, the final numerical score and letter grade displayed on the Safety Grade Review Website during the 1-week Letter Grade Embargo Period. File also includes national benchmarking scores for each measure and is sent out with the Safety Grade Calculator. 

Review final numerical scores and Safety Grades, track changes in measure scores and grades between rounds, compare performance on measures, domains, and final grades among your hospitals and nationally. Use the z-score for each measure to identify opportunities for improvement and the impact of individual measures on the overall Safety Grade. 

Competitive Benchmarking Scores

Competitive Benchmarking Summary Scores File – Excel workbook containing a data dictionary, individual hospital Domain Scores for Inpatient Care Management, Medication Safety, Infections, Maternity Care, Pediatric Care, Complex Surgery, and Outpatient Procedures, individual hospital Value Scores, and national average scores for each domain and the national average Value Score. Preliminary Summary Scores are based on Surveys submitted by August 31. Year-End Summary Scores are based on Surveys finalized by January 31.

 

Health Systems can also receive a discount on Competitive Benchmarking Reports. More information about the reports is available here.

The Domain and Value Scores summarize hospital performance on all measures included in the Leapfrog Hospital Survey and can be used to benchmark overall performance on the Survey, to engage service lines and departments, and to educate boards on areas of achievement and opportunities for improvement. Value scores are one component of Leapfrog's Top Hospital methodology.
