St. Bernard, a safety-net hospital on the South Side of Chicago, was shocked to realize they had earned an “F” on The Leapfrog Group Hospital Safety Grade in 2021. CEO Charles Holland and Chief Quality and Patient Safety Officer Michael Richardson used the Leapfrog Hospital Survey to identify areas for improvement and eventually improved from that “F” to an “A” grade in spring 2023.
