The 2024 Leapfrog Hospital Survey is free and open to hospitals from April 1 to November 30. The annual survey assesses hospital safety, quality, and efficiency based on national performance measures that are of specific interest to health care purchasers and consumers. Survey Results are publicly reported, and provide hospitals with information to benchmark their progress in improving the care they deliver. For general, acute-care hospitals only, some Survey Results are also used to calculate the Leapfrog Hospital Safety Grade.
January 31 Deadline Reminder
Hospitals that need to correct data entry or reporting errors (i.e., in response to Leapfrog’s Extensive Monthly Data Review) to previously submitted sections of the 2024 Leapfrog Hospital Survey must make necessary corrections and re-submit the entire Survey by January 31, 2025. Hospitals will not be able to make changes or re-submit their Survey after this date.

Alignment with Other National Programs
Many of the measures included in the Leapfrog Hospital Survey are in use by other national agencies and organizations including the Centers for Medicare & Medicaid Services (CMS), the CDC's National Healthcare Safety Network (NHSN), and The Joint Commission. For general hospitals, some of the measures are used to calculate the Hospital Safety Grade.

Leapfrog's Rating Programs
Leapfrog administers two different ratings programs for hospitals: the Leapfrog Hospital Survey and the Leapfrog Hospital Safety Grade. Most every hospital is targeted to submit an annual Hospital Survey, but only general hospitals are eligible for a Hospital Safety Grade.
